What Is the Referral Portal?
The Vertex Referral Portal is a digital intake and referral management solution designed for the IDD industry.
Referral partners, including supported employment programs, social services departments, vocational rehabilitation agencies, and community organizations, can access your organization’s referral form directly from your website. They complete the form with the consumer’s information, service needs, and relevant background. That submission is routed through the Vertex platform, reviewed and approved by your team, and then used to automatically create the consumer’s record in Case Manager.

How the Referral Portal Works

Vertex builds your referral form based on the intake information your organization needs. The completed form is embedded on your website. Referring partners visit your site, open the form, and fill it out with the consumer's demographic information, service needs, funding source, and any relevant background.
Submission and ReviewÂ
Every time a partner opens the form, a new referral is created. Submitted referrals are routed to your team for review inside Vertex. Staff can evaluate the referral, request additional information if needed, and approve or decline based on your organization's capacity and criteria.
Automatic Record CreationÂ
When your team approves a referral, the consumer record is automatically created in Vertex Case Manager. The data submitted by the referring partner populates the record directly. No second step. No re-entry. No transcription errors.
